- Task No. 1 Delivery of a diagnostic and therapeutic device for the rehabilitation of upper limbs 1. The subject of the order is the delivery by the Contractor of a diagnostic and therapeutic device for the rehabilitation of upper limbs (hereinafter: equipment), described in detail in the List of Required Functions and Parameters Technical constituting Annex No. 6 to the SWZ, in the quantities indicated in the price form constituting Annex No. 2 to the SWZ and in accordance with the Draft Provisions of the Agreement constituting Annex No. 4 to the SWZ.2. The purchase by the Ordering Party of the equipment referred to in section 1 will be made as part of the agreement concluded The State Treasury and the Minister of Health signed a contract for the implementation of a multi-annual program called National Oncology Strategy" in the scope of "Purchase of equipment for therapeutic rehabilitation for oncology patients in 2025" (hereinafter referred to as the subsidy agreement). 3.
