Description:
1. The subject of the order is the delivery of Gamma SPECT/CT cameras with equipment and the performance of adaptation works.
2. A detailed description of the subject of the order is included in Annex No. 1a to the SWZ, the scope of adaptation works is included in Annex No. 1c to the SWZ.
3.Subject matter and conditions of implementation of this orders should be in accordance with the Act of 7 April 2022 on medical devices and the Regulation (EU) 2017/745 of the European Parliament and of the Council of 5 April 2017 on medical devices or the Regulation (EU) 2017/746 of the European Parliament and of the Council of 5 April 2017. on in vitro diagnostic medical devices (if applicable) and other applicable legal provisions in this regard. The device offered must have a CE declaration.
4. The device must be approved for marketing and use in the Republic of Poland in service units health, in accordance with applicable law (declaration of conformity for the entire device, CE marking, notification or entry of the medical device to the President of the Office for Registration of Medicinal Products, Medical Devices and Biocidal Products).
5. The subject of the order must be complete and ready for use after unpacking, installing, running without any additional purchases or investments. The subject of the order should be produced in 2025.
