Description:
1. The subject of the order is the supply of a hydraulic urethral sphincter and a disposable anastomat for circumcision for UCK in the assortment and the requirements specified in Annex 3 to the SWZ.
2. The Ordering Party requires that the offered assortment constituting a medical device meets the requirements concerninge of medical devices contained in the Act of 7 April 2022 on medical devices (i.e. Journal of Laws of 2022, item 974, as amended) and in executive acts issued on the basis of this act.
3. Medical devices must meet the requirements set out in Appendix No. 3 to the SWZ. Even if it is not fulfilledof any of the requirements set out in Annex 3 to the SWZ will result in the rejection of the offer.
4. Medical devices referred to in par. 1 The Contractor will be obliged to deliver at his own expense to the Medical Supply Department, ul. Smoluchowskiego 17.
5. The delivery of medical devices under this order will becarried out on the basis of partial orders. Maximum delivery time is 6 business days
