Description:
1. The subject of the order is the supply of hysteroscopic instruments for the Department of Obstetrics and Gynecology for USK-2 in Szczecin (hereinafter referred to as the products). 2. Estimated quantities of products and a description of the products together with the necessary requirements are included in the offer form (Task 1 to SWZ, hereinafter also referred to as FO), price form unit (annex 2 to SWZ, hereinafter also referred to as FCJ), and the required parameters form (annex 2A to SWZ, hereinafter also referred to as FPW). 3. The Ordering Party requires the offer of medical devices within the meaning of the Act of April 7, 2022 on medical devices (Journal of Laws 2024.1620, as amended), hereinafter referred to as: UoWM), introduced to trade and use in the territory of the Republic of Poland in accordance with applicable regulations. ATTENTION! EXCEPT FOR ITEMS NO. 9, 10, 11, 124. The ordering party requires that the delivered products have a warranty period of no less than 24 months, counting from the date of their delivery to the ordering party's office. The warranty period is one of the criteria evaluation of offers. Detailed warranty conditions are described in Chapter II of the SWZ Draft Provisions of the Agreement. 5.
