Description:
The subject of the contract is:
1) delivery with a placement to the patient's home address indicated by the Dialysis station indicated by the Dialysis Station, the peritoneal dialysis sets and devices for heating and supply of dialysis fluid in accordance with Annex 2For part 1 and 2,
2) delivery along with bringing to the hospital pharmacy of the Provincial Specialist Hospital Janusz Korczak in Słupsk Sp. z o. o. at ul. Hubalczyk 1, dialysis fluids in accordance with Annex 2 of the assortment and quantity form for parts 3 and 4,
3) delivery along withbringing to the hospital pharmacy of the Provincial Specialist Hospital Janusz Korczak in Słupsk Sp. z o. o. at ul. Hubalczyk 1, blood line, needles for hemodialysis and dialysis, in accordance with Annex 2, the assortment and quantity form for parts 5 and 6,
4) lease on timethe duration of the apparatus necessary to perform ADO procedures in a total amount of 17 pieces along with the training of staff in the field of equipment handling confirmed the receipt protocol, as well as patients to which the equipment will be transferred - applies to part No. 1.
