Description:
1. The subject of the order is the supply of reagents and consumables for performing immunochemical determinations, along with the lease of an analyzer for their performance.2. Ordering Party's requirements:2.1. The contractor is obliged to deliver and install the analyzer to the hospital laboratory at ul. Krychnowicka 1, 26-607 Radom and staff training until December 22, 2025.2.2. The contractor will connect to the LIS Zelnet IT network by December 30, 2025.2.3. Reagents delivery time: max. 5 business days from placing the order.2.4. The contractor will provide warranty service for the analyzer during the contract period.3. A detailed description of the subject of the order is provided: 3.1. Appendix No. 2 to SWZ Price form; 3.2. Appendix No. 1 to SWZ Minimum technical requirements of the analyzer;3.3. draft provisions of the contract, Annex No. 4 to the SWZ;3.4. the ordering party requires that the subject of the order listed in Annexes 1 and 2 be a product medical device in accordance with the requirements specified in the Act of April 7, 2022 on medical devices, has the CE mark and current documents authorizing marketing and use in Poland.
