Description:
1. The subject of the contract is successive deliveries: implants and orthopedic endoprostheses, including tiles and bolts for the treatment of bone fractures, metacarpal nails, endoprostheses, cement and non -cementless and walks. According to the assortment presented in detail in Annex 1a to SWZ - (Asor formTEMPERNIAL -PRICE), which provided an estimated number of products systematized in 6 packages.
2. The commissioner requires:
a) the contractor only offered medical devices, which were allowed for trading and use in accordance with the requirements of the Act of April 7, 2022. About Medic deviceszne (i.e., Journal of Laws of 2024, item 1620, as amended) and executive provisions to this Act. Documents for inspection at each request of the Ordering Party at each stage of the contract.
B) The subject of the contract must meet the qualitative requirements appropriate for each of them, packaging requirements (CE mark on OPAstorage) and storage and have permission to marketing (CE declarations, CE certificates - if required. Documents for inspection at each request of the Ordering Party at each stage of the contract.
c) On the unit packaging of the subject of the contract, the trade label must be placedWA prepared in Polish containing the necessary information: commercial name, catalog number as well as the series and expiry date, before the expiry of which the product can be safely used and GTIN (static part of the UDI code).
