Description:
1. The subject of this order is the delivery of implants for the Lower Silesian Center of Rehabilitation and Orthopedics Sp. z o.o. in Kamienna Gora at ul. J. Korczak 1.
2. The subject of the contract was divided into five (5) parts:
part 1ndoprosthesis of the hip joint
part 2ndoprosthesis non -ferturehip joint
part 3ndoprosthesis of the knee
part 4 latch padders
part 5 Bone cement
3. The setting description of the subject of the contract is contained in part II SWZ (subject of the order) and in Annexes No. 1.1 - 1.5 to the offer form.
4.Approximately and may change depending on the real needs of the Ordering Party. Quantities may be reduced, as the contracting authority will make orders according to current and actual needs and their final quantity may change in relation to the quantities specified in Part II SWZ, withReduction of the order may not exceed 30% of the total value of the subject of the contract. Failure to be applied during the period of this Product Agreement in the quantity specified in Annexes 1.1 1.5 to the offer form (in the scope of individual parts), however, no more than 30% of the total valuethe subject of the contract, does not constitute grounds for the contractor to pursue claims for the order of other products or other financial claims in this respect.Obow ...
