- The subject of the order is the supply of infusion fluids
The subject of the order was divided into 12 tasks - in accordance with Annex No. 1 to the SWZ "Assortment and price form".
Detailed description and parameters of the product, required form/volume, unit of measurement, quantity are specified in the "Assortment and price form constituting Annex No. 1 to the SWZ.
Detailed conditions and rules for the implementation of the subject of the order are specified in the provisions of the Contract Template - Annex No. 5 to the SWZ "Contract Template".
Series and expiration date on the unit packaging and on the invoice.
The validity period of the offered subject of the order, not shorter than 12 months from the date of delivery to the Ordering Party.
The contractor will deliver the subject of the order to the HOSPITAL PHARMACY Warehouse.
The offered product must be authorized for marketing and use as a medicinal product for each required form/volume of the drug separately in accordance with the conditions specified in the Act of September 6, 2001 - Pharmaceutical Law (consolidated text: Journal of Laws of 2025, item 750).
3 must be approved for circulation and use in the territory of the Republic of Poland in accordance with the Act
on Medical Devices (consolidated text: Journal of Laws of 2024, item 1620).
