The subject of the order is the delivery, installation, 4 pieces of devices for active regulation of the patient's body temperature and 9 pieces of devices for heating infusion fluids for the Integrated Operating Theater USK-2, hereinafter referred to as equipment.2. The description of the subject of the order is included in the Parameters form required (attachment 2 to the SWZ, hereinafter also referred to as FPW), this form should absolutely be submitted together with the offer form (attachment 1 to the SWZ, hereinafter also referred to as FO).3. The Ordering Party requires that the offered equipment constitutes a medical device within the meaning of the Act of April 7, 2022 on medical devices (i.e. Journal Laws of 2024, item 1620 from Mon. amended, hereinafter referred to as: UoWM), placed on the market and used in the territory of the Republic of Poland in accordance with applicable regulations. 4. Ordering party pursuant to Art. 30 section 2 of the Public Procurement Act, awards the contract in parts, each of which is the subject of separate proceedings.5. The order consists of 2 tasks: a) Task No. 1 Device for active regulation of the patient's body temperature - 4 pieces; b) Task No. 2 - Device for heating infusion fluids - 9 pieces; The Ordering Party allows the submission of partial offers covering individual tasks. ...
