Description:
Delivery of laboratory and bacteriological tests and reagents, including equipment rental:
biochemical reagents,
Hematology reagents,
Immunochemical reagents,
coagulation reagents,
Reagents for urine analysis,
Microbiological reagents,
Serological reagents.
1.Detailed description the subject of the order, as well as each part of the order, is included in Annex No. 2 to the SWZ. The Ordering Party reserves the right to reduce the delivery specified in the annex to the offer by 35%, as well as to suspend deliveries of some reagents, in particular due to the variability of the quantities of diagnosed patients and the size of contracts signed with the National Health Fund.
2. The supplied reagents and leased equipment must be approved for trading in the Republic of Poland and for use in health care facilities and comply with the standards in force in the European Union (CE mark, declaration of conformity).
3.Reagents provided should have information containing the expiry date and other necessary data, including, in Polish, a written safety data sheet for hazardous substances and dangerous preparations, if the reagent contains hazardous substances, as well as information and instructions regarding warehousing and storage.
4. The Contractor guarantees that the delivered reagents are of high quality and that they will have an expiration date in accordance with the Contractor's offer, not shorter than 6 months from the date of delivery, unless the Ordering Party has allowed a shorter period in Annex No. 2 to SWZ.
