Description:
1. The subject of the contract is the supply of medical devices for electrophysiological diagnostics, ablation and one -off medical devices for Copernicus PL Sp. z o.o., including for equipment and cameras in the equipment of the Employer. The subject of the order consists of 69 parts.
2. Detailed The scope and description of the subject of the contract, estimated quantities and manner of processing the order include attachments to the specification of the terms of the contract (hereinafter referred to as SWZ), namely:
No. 1-assortment and price form,
No. 3 - contract projects.
Annex 1 to SWZ is attached to the offer. The contractor, by completing the indicated items in Annex 1 to SWZ, confirms the meeting of the requirements specified by the Employer.
3. The performance of the subject of the contract is determined by contract projects- Annex 3 to SWZ, namely: draft contract- a commission without lending (regarding parts 3, 6, 7, 9, 10, 62), Design of the contract-sales (regarding parts 8, 11, 13, 14, 16-25, 26-61, 63-69), draft contract-sale with lending (regarding part 15), draft contract-commission without lending + lease (regarding parts 1, 2, 4, 5, 12).
4. The maximum partial order delivery date is up to 5 business days (including Further SWZ entries - the delivery date is a criterion for evaluation of offers).
5. The order items must be complete, with a high standard, both in terms of quality and functionality, as well as free from material, construction and legal defects.
