- 1. The subject of the contract is: delivery of medical devices, including antiseptic agents, dressings and patient care products, and vascular protecting agents for the needs of the WSK-2 PUM clinics. Description of the subject of the contract and the estimated quantities regarding the products have been included in the unit price form (Annex 2 to SWZ, hereinafter also as FCj)
2. The resort requires offer
and). medical devices (applies to all tasks, all items, excluding items 7-10 in Zad 3, in which the Employer requires the offer of cosmetics)-within the meaning of the Act on medical devices (Journal of Laws 2024.1620, as amended), hereinafter: UOWM), placed on the market and use on the territory of the Republic of Poland in accordance with applicable regulations.
3. The applicant requires that the shelf life for all offered products be a minimum of 12 months from the date of delivery to the headquarters of the Ordering Party.
4. It has the right to order a smaller amount of products, but not less than 60% of the amount given in the unit price form.
5. Meeting consists of 4 tasks whose detailed description was included in the FCJ
6. The appointment does not allow the submission of variant offers.
