Description:
1. The subject of this order is the supply of medical devices for the National Oncology Institute of Maria Skłodowska - Curie -State Research Institute of the Branch in Gliwice
2. The subject of the contract is divided into 2 separate tasks.
3. Description of the subject of the contract and conditions The implementation of the order is included in the attachments:
1) technical parameters, assortment and price specification-Annex 2.1-2.2 to SWZ,
2) the planned provisions of the contract, which will be entered into the content of the contract - Annex 4 to SWZ.
4. The contractor must join the offer The subjective measures in question, confirming that the offered subject of the contract corresponds to the requirements specified by the Ordering Party: declaration of compliance and/ or CE certificate (depending on the class of medical device) and relevant statements (if required) in accordance with the 2017/745 regulation in the 2017/745 regulation matter of medical devices (MDR) or with the Council Directive 93/42/EEC (MDD) together with later transitional provisions.
2 above they constitute the subjective measures to confirm that the offered subject of the contract corresponds to the specific requirements by the contracting authority.
107 para. 2 of the Act, the Employer informs that the subjective measures referred to in point 2 shall be supplemented if they are not submitted with the offer or are incomplete.
