- 1. The subject of the contract is the delivery of medical devices to URS together with the rental of the image converter to URS to the Ordering Party's headquarters (31-826 Krakow, Os. Golden Autumn 1), as described and the requirements contained in Annex 1A and 1B to NIN. Specifications.The subject of the contract may not be shorter than 12 months from the date of delivery.
3. Imaging on medical devices to URS:
a) The Contractor undertakes to conduct the ordering personnel at the request of the contracting authority at his headquarters - os. Golden Autumn 1, 31-826 Krakow withThe first delivery of the goods to the hospital pharmacy within the period agreed in writing with the Employer, but not later than within 30 days from the date of the contract.
b) the Contractor will be obliged to submit a attendance list from the training, referred to in para. 3 lit. and with the signatures of trainedusers confirming their actual presence to the hospital pharmacy, within 5 business days from the date of the training.
4. Imaging regarding the image converter to URS:
a) The Contractor is obliged on the day of delivery of the subject of the contract to train employees of the Ordering Party withdelivered devices at the headquarters of the Ordering Party (31-826 Krakow, Os. Golden autumn 1) or ensuring at your own expense such training by a third party, which will be confirmed by the issue of personal certificatesconverted ... - Documents:
