Description:
The subject of the order is the delivery of a lamp emitting polarized light along with its installation, commissioning and conducting the necessary training - 2 sets.1. The medical device offered by the Contractor should have valid approvals for marketing and use in the territory of the Republic of Poland. Poland, in accordance with Polish law and European Union law, and to confirm, the Contractor will submit a declaration that he has a set of documents allowing the offered subject of the order to be traded in the territory of the Republic of Poland in accordance with the Act of April 7, 2022 on medical devices (Journal of Laws 2024, item 1620) and that, at each request of the Ordering Party, it will provide certified photocopies of these documents.2. The offered medical device must meet the requirements (standards, parameters) specified by the Ordering Party in the identification and price form.3. Offered medical device must be brand new with a production date no older than 2025.4. The offered medical device must be free from any physical and legal defects.
