Description:
The subject of the order is: Delivery of medical equipment Task 1 - Clamp for aortic aneurysms for the Cardiac Surgery Operating Room Task 2 - Aortic clamps for the Cardiac Surgery Operating Room The Ordering Party allows the submission of partial offers. In parts orders are individual tasks specified above, i.e. tasks 1 to 2. The Ordering Party informs that the Contractor may submit an offer for a maximum of 2 tasks. (without limiting the number of tasks per contractor). A detailed description of the subject of the order is included Annex 2b and Annex 4 to the SWZ. 3.3. The Ordering Party's requirements regarding the subject of the order: 1) the offered medical device must have approvals, certificates and be registered in accordance with with legal regulations requiring approvals, certificates and registration, 2) the offered medical device must be placed on the market and used in accordance with with the requirements of the Act of April 7, 2022 on medical devices (Journal of Laws of 2024, item 1620, as amended), 3) the offered medical device must meet the standards or technical specifications applicable to this product; If the offered medical product does not require notification to the President of the Office for Registration of Medicinal Products, Medical Devices and Biocidal Products, the Contractor is obliged to perform at its own expense and on its own, all activities necessary to fulfill all legal requirements related to placing the product on the market and for use in the territory of the Republic of Poland.
