Description:
1. The subject of the contract is the delivery of medical equipment to the headquarters of the Ordering Party in accordance with the description and requirements contained in Annexes No. 1a-1D to the specification (in terms of the part for which the offer is submitted).

2. 2nd release (applies to all packages); Installation, installation and launch (if concerned) equipment at the Specialist Hospital Ludwika Rydygiera in Krakow sp.z o.o., 31-826Krakow, os. Golden Autumn 1.
3. The contractor undertakes on the day of delivery of the subject of the contract to conduct the training of the ordering person's personnel (to the extent described in the Annex 1A-1D of the specification) at his headquarters-os. Golden Autumn 1, Krakow or assurances at your own expenseSuch training by a third party. The training will be confirmed by the issue of personal certificates.
Offer placed for the entire order must be secured with a bid bond in the amount of: PLN 85,900.00 (in words: eighty -five thousand nine hundred zlotys 00/100).
