Description:
The subject of the order is:
Delivery of medical equipment and other medical materials
Task 1 Ambulatory ECG/ICG monitor for the 3rd Department of Cardiology
Task 2 Triggerfish ocular lenses with a built-in measuring microscope for the 3rd Department of Cardiology
Task 3 Camera and microphone directional for the 3rd Department of Cardiology
The Ordering Party allows the submission of partial offers.
Parts of the order are the individual tasks specified above, i.e. tasks 1 to 3.
The Ordering Party informs that the Contractor may submit an offer for a maximum of 3 tasks.
(without limiting the number of tasks per one Contractor).
A detailed description of the subject of the order is included in Annex 2b and 5 to the SWZ.
3.3. The Ordering Party's requirements regarding the subject of the order (if applicable):
1) the offered medical device must have approvals, certificates and be registered in accordance with
with legal regulations requiring certificates, certificates and registration,
2) the offered medical device must be placed on the market and used in accordance with
with the requirements of the Act of April 7, 2022 on medical devices (Journal of Laws of 2024, item 1620, as amended),
3) the offered medical device must meet standards or technical specifications applicable to this product;
If the offered medical product does not require notification to the President of the Office for Registration of Medicinal Products, Medical Devices and Biocidal Products, the Contractor is obliged to perform, at its own expense and on its own, all activities...
