Description:
1. The subject of the contract is the successive delivery of medical equipment to the operating theater for the needs of "Pro-Medica" in Ełk Sp. z o.o. ', number 2152/2025. A detailed description of the subject of the contract (OPZ) contains an assortment and price form, constituting Annex 3 to SWZ.
2. The Ordering Party allows submitting partial offers (package 1-16).
3. In addition, the subject of the contract is:
1) (package 1) lease 1 items of reusable electrically controlled use,
2) (package 8) lease 2 pcs
3) (package 9) lease of 1 pcs of a generator with vessel closing system,
4) (Package 13) lease of 1 pcs of two energy generator.
The above procedure is the 2nd stage of the planned 2 stages in 2025.
5. The scope of the contractor's duties and the manner of execution of the order It was specified in detail in Annex 5 to SWZ - planned provisions of the contract.
6. The subject of the contract must meet the requirements specified in the Act of 07.04.2022 on medical devices (i.e., Journal of Laws of 2024, item 1620), must have documents allowing and use in Poland, in particular to be marked with the CE mark specifying compliance with the requirements of the European Union.
7. The contracting authority requires that the offered products meet the requirements specified in applicable law, be allowed to trade and have valid documents required by law, stating permit ...
