The subject of the order is the supply of medical equipment for the Regional Center of Dentistry of the Provincial Hospital. L. Rydygiera in Toruń, divided into 5 tasks.
Task No. 1 - dental units with accessories for the Pediatric Dentistry Clinic - 2 pieces.
List of requirementsThese technical and operational parameters for the offered subject of the order are specified in the Annex. No. 3/1 to SWZ.
The Contractor calculates the offer price including the goods and services tax (VAT) by completing the price specification template - Attachment. No. 2/1 to the SWZ, depending on the type and quantity of equipment covered by the data fromadan.
The Contractor is obliged to attach these forms to the offer.
Task No. 2 - dental units with accessories for the Conservative Dentistry Clinic - 2 pcs.
The list of required technical and operational parameters for the offered subject of the order is specified in Attachment. No. 3/2 to SWZ.
Contractor of calculationstotals the offer price including the goods and services tax (VAT) by completing the price specification template - Attachment. No. 2/2 to SWZ, depending on the type and quantity of equipment involved in a given task.
The Contractor is obliged to attach these forms to the offer.
