• Task No. 1: Delivery of liquid medical oxygen with tank lease Task No. 2: Delivery of gases for cryotherapy along with the lease of tanks and cylinders Task No. 3: Supplies of medical gas for anesthesia 50/50% O2/N2O along with the lease of cylinders, trolleys and valves dosing Task No. 4: Delivery of special gases with the lease of cylinders Task No. 5: Delivery of medical gases with the lease of cylinders Task No. 6: Supplies of gases for medical purposes, including the lease of cylinders 1. The subject of the order is the successive deliveries of gases medical products (hereinafter interchangeably referred to as gases or goods), the type and quantity of which were specified in the price form constituting Annex No. 2 to the SWZ, and successive deliveries of anesthetic gas equipment (50% - oxygen / 50% - nitrous oxide), i.e. mouthpieces - applies to task no. 3 (hereinafter referred to interchangeably as the goods) in the quantity given in the price form constituting Annex 2 to the SWZ together with the lease: stationary tank with evaporators for liquid oxygen (concerns task no. 1), nitrogen tank liquid and liquid air tank for cryotherapy (applies to task 2), cylinders (applies to tasks no. 2, 3, 4, 5, 6), and accessories, i.e. cylinder trolleys and dosing valves (applies to task no. 3) (hereinafter interchangeably referred to as the subject of the lease), in detail described in the price form constituting Annex No. 1 to the contract, as well as other obligations arising from the contract aimed at enabling the Ordering Party to use the goods and the subject of the lease...
