Description:
The subject of the order is the successive delivery of medical materials to the Ordering Party's hospital pharmacy. A detailed description of the subject of the order, requirements as well as estimated quantities and type of assortment are specified in Annex No. 1 to the SWZ, which is also the forms assortment and prices.

3. The subject of the order should be admitted to trading in the territory of Poland and have markings consistent with the description contained in legal acts:
- for a medicinal product in the Act of September 6, 2001 r. Pharmaceutical Law (consolidated text: Journal of Laws of 2025, item 750, as amended),
- for medical devices in the Act of April 7, 2022 on medical devices (consolidated text: Journal of Laws of 2024, item 1620).
4. The quantity specified in Annex 1 to the SWZ is an estimated amount, depending on the number of admitted patients, available financial resources and the content of negotiated contracts with the National Health Fund. Based on the above-mentioned premises, the estimated quantities specified in Annex No. 1 to the SWZ may be reduced in accordance with the actual current needs of the Ordering Party.
