The subject of the order is the supply of medical devices with accessories for the Ophthalmology Department, in accordance with the product and price list constituting Annex No. 2 to the SWZ and the required technical parameters for the phacoemulsification device and anterior and posterior vitrectomy, parametertechnical parameters for the device for surgical freezing of tissues together with the accessories, technical parameters for the tonometer, technical parameters for the computer perimeter, constituting Annex No. 3 to the SWZ and warranty and service conditions for the device for surgical freezing of tissuestogether with instrumentation, tonometer, computer perimeter, constituting Annex No. 4 to SWZ
2. The Ordering Party allows the submission of offers for parts:
Lot No. 1 - supply of consumable accessories, compatible with the offered device for phacoemulsification and anterior and posterior vitrectomy along with accessoriescamera jaw
Part No. 2 - delivery of one-piece and three-piece foldable intraocular lenses
Part No. 3 - delivery of intraocular preparations
Part No. 4 - delivery of intracapsular rings in a disposable applicator
