Description:
1. The subject of the contract is the supply of medical, specialist and one -off equipment. The subject of the order was divided into 82 parts. Offers can be submitted in relation to one, several or all parts of the order. The offer in each part should be full and should meet the detailed Requirements specified in SWZ and its attachments. A detailed description and method of order processing includes SWZ, in particular Chapter IV para. 7 and 8, price form, constituting Annex 1A to SWZ and templates of contracts, constituting Annexes 5a, 5B, 5C, 5D to SWZ.
2. Subjective means:
1) Employer requires submitting with the offer N/in the means of evidence:
106 para. of the Act on Public Procurement Law, confirming the parameters confirming the compliance of the offered equipment with the requirements of the price form - applies to part No. 13, 22, 25, 37, 62, 68;
b) Certificate certifying the manufacturer's possession of ISO 13485 or an equivalent document for the offered products medical (in the case of an equivalent standard, such information should be included in the offer) with the exception of part 4 points. 2 and points 3; 15; 16; 20a; 20b; 44A; 49a;
c) certificate confirming the manufacturer's possession of ISO 14001 or an equivalent document for the offered medical devices (in the case of equivalent, such information should be included in the offer) for part 43, 46, 70;
