- Title: DELIVERY OF MEDICATIONS Description: 1. The subject of the order is the "Supply of medicinal products under drug programs" in accordance with the description and requirements contained in Annexes No. 2/2.12 (Excel sheet) to the Specification (in the assortment and price forms). 2.By the medicinal product constituting the subject matter orders should be understood as medicinal products within the meaning of the Pharmaceutical Law Act of 6 September 2001 - Pharmaceutical Law (Journal of Laws 2025.750, consolidated text of 2025/06/06). The offered medicinal product must be authorized for marketing under the principles set out in Art. 3 or 4 section 1 and 2 or 4a of the Act Pharmaceutical law. 3. If it is found that the chemical substance in the dose and form indicated in the SWZ is included in the Announcement of the Minister of Health on the list of reimbursed medicines, foodstuffs intended for particular nutritional uses and products as at the date of submission of offers. medical - Annex B and/or C, and the offered medicinal product is not listed there, the offer will be rejected. The offer will also be rejected if the price of the offered medicinal product is higher than the official price indicated in the Notice of the Minister of Health. 4.All products offered must meet the requirements described in the Specification. 5. Offers that do not include the full scope of the subject matter of the contract described in this ToR will be rejected.
