Description:
The subject of the order is the successive delivery of the medicinal product - Verteporphyrin. The order is not divided into parts. The agreement will be concluded after the completion of these proceedings for a period of 12 months. The ordering party did not divide the order into parts. The subject of the order includes delivery of one, precisely defined medicinal product - verteporphyrin (verteporfin) in the form and dose authorized for marketing in the territory of the Republic of Poland. This medicine is a reference product and has no generic equivalents or therapeutic substitutes approved in Poland. Due to lack of equivalent preparations, dividing the order into parts is not possible. The lack of division of the order into parts does not limit competitiveness in relation to small and medium-sized enterprises.
A detailed description of the subject of the order and a description of the ordering party's requirements in the scope of deliveries are specified in: - The delivery specification is included in Appendix No. 1 to the SWZ ("Offer Form"), and the detailed characteristics are included in Appendix No. 1.1 to the SWZ ("Price Form"). - Draft provisions of the contract - Annex No. 2 of the SWZ.
