Description:
The subject of the order is the successive delivery of medicinal products, contrast agents, renal therapy products and nutritional products for the needs of organizational units subordinated to SPZOZ in Myszkow. The order is divided into 40 packages (parts).
The delivery specifications are included in the annex No. 1.1 to the Terms of Order Specification ("Price Form"). The remaining terms of the contract are specified in Annex No. 2 to the terms of reference ("Proposed provisions of the contract regarding public procurement, which will be introduced into the content of the contract"). Agreements regarding individual packages will be concluded after the completion of these proceedings for a period of 12 months.
The offer consists of the Offer Form - Appendix No. 1 to the SWZ and Price form - completed in the scope of the package/packages for which the offer is submitted - Annex No. 1.1 to the SWZ. The offer includes join:
