Delivery of medicinal products financed by the National Health Fund under Drug Programs, Chemotherapy, Emergency Access to Drug Technologies and hematopoietic cell transplantation procedures 67 tasks.
1. The subject of the order is the supply of medicinal products financed by the National Health Fund under the Drug Programs, Chemotherapy, Emergency Access to Drug Technologies and the procedure hematopoietic cell transplantation.
The subject of the contract is 67 tasks.
3. The validity period of the subject of the order (suitability for use) must be at least 12 months from the date of delivery to the Ordering Party's registered office.
4. The contractor ensures labeling of the medicinal product in accordance with the Regulation of the Minister of Health of February 20, 2009.
on requirements regarding the labeling of medicinal products and the content of leaflets (Journal of Laws 2020, item 1847).
5. The Ordering Party requires the Contractor to include the EAN code of the offered medicinal product in its offer (if given).
6. The Ordering Party requires that the offered medicinal products be included in the Announcement of the Minister of Health (so-called reimbursement) regarding the financing of drug and chemotherapy programs (if applicable).
