Description:
1. The subject of the contract is "Delivery of medicinal products as part of drug programs" as described and the requirements contained in Annexes 2 / 2.9 (Excel sheet) for specification (in assortment and price forms). 2. A medical product, which is the subject of the contract, should be understood Medicinal products within the meaning of the Pharmaceutical Law of September 6, 2001 - Pharmaceutical Law (Journal of Laws 2015.750, i.e. of 2025.06.06). The offered medicinal product must be approved for trading on the principles set out in art. 3 or 4 para. 1 and 2 or 4a of the Pharmaceutical Law.
3. on In the event that the chemical in the dose and form indicated in SWZ is found in the current announcement of the Minister of Health on the list of reimbursed drugs, foods of special nutritional purpose and medical devices - Annex B - I/or C, and the offered medicinal product is not demonstrated there, the offer will be rejected. The offer will also be rejected when the price of the offered medicinal product is higher than the official price indicated in the announcement of the Minister of Health. 4. All offered products I have to meet the requirements described in the specification. 5. Openses that do not contain the full scope of the subject of the contract described in this SWZ will be rejected.
