Description:
1. The subject of the contract is the supply of medicinal products used in drug programs and chemotherapy for cancer for the needs of the WSK-2 PUM clinics. Description of the subject of the contract and estimated amounts of products were included in the unit price form (annex 2 to SWZ, hereinafter also asFCj).
2. Among the offer requires the offer of medicinal products in accordance with the Act of 6 September 2001 Pharmaceutical Law (Journal of Laws 2001.126.1381 as amended), placed on the market and use on the territory of the Republic of Poland in accordance with applicable regulations.

- Annex B to the announcement of MZ on the list of reimbursed medicines - applies to the following tasks: 1, 2, 3, 4 item 1-3; 5, 6, 7, 9, 10, 11
- Annex C for the announcement of MZ on the list of reimbursed medicines - applies to the following tasks: 4 item 4; 8, and the price offeredIt may not exceed the financing limit for a given active substance.
