The subject of the order is the successive delivery of medicines, foodstuffs for particular nutritional uses and medical products for the Provincial Hospital of... Dr. Roman Ostrzycki in Konin.
The subject of the order is described in detail in Annex 2 to the SWZ (Assortment and price form), which provides the quantity of the subject of the order and a description of the subject of the order for the basic order and for the order covered by the option right. The Ordering Party allows for the submission of partial offers. The subject of the order was divided into 100 parts (packages). The Ordering Party allows submitting an offer for one, several or all parts. The subject of the order will be delivered at the expense, risk and transport of the Contractor. The conditions for the implementation of the Subject of the Order are also included in the draft provisions of the contract, constituting Annex No 5 to SWZ. The minimum guaranteed level of the value of orders that the Ordering Party undertakes to place (minimum guaranteed level of use of the contract value) is specified in the Draft Provisions of the Agreement constituting Annex No. 5 to the SWZ.
