Description:
The order has been divided into 6 packages.
1. The removal to confirm the compliance of the offered supplies requests the submission of the evidence listed below in question:
1) a statement that the offered medicinal products are approved for marketing on the territory of the CommonwealthPolish and have current registration certificates, in accordance with the provisions of the Act of 06.09.2001 - Pharmaceutical Law and will be made available at each request of the Ordering Party after signing the contract - the content of the statement contained in the offer form constituting Annex 1 to SWZ.healing.
2) a statement that the offered assortment has been allowed and placed on the market, in accordance with the provisions of the Act of 7 April 2022 on medical supplies and has current declarations of compliance and certificates of notified unit and they will be made available at any requestThe Ordering Party after signing the contract - the content of the statement contained in the offer form, constituting Annex 1 to SWZ. -Dotych medical devices.
3) a statement that the offered medicinal products have the required characteristics cards and will be submitted to each call of the Ordering Party aftersigning the contract - the content of the statement contained in the offer form constituting Annex 1 to SWZ.
