Description:
The subject of the contract are successive delivery by the contractor of a one -off assortment for the cardiac surgery department for a period of 36 months, hereinafter contracts constituting Annex 4 to this SWZ.
In tasks No. 7, 8, 9, 10 as part of the implementation of this Agreement, during its validity, the Contractor will lend the Employer Holder after 1 piece in each task.
The contracting authority informs that in the subject proceedings under the tender Unlimited provides for the possibility of applying the procedure referred to in art. 139 para. 1 of the Act The contracting authority, using the above -mentioned procedure the scope of no grounds for exclusion and compliance with the conditions of participation
in the proceedings.
Task No. 1: Delivery of rings for valve cancellastics mitral and trigeum
1. The subject of the contract is successive delivery by the contractor of the rings for mitral and trigeum valve cancellastics For Cardiac Surgery Department for a period of 36 months, hereinafter referred to as interchangeably, described in detail in the content of the price form constituting Annex 2 to SWZ and in accordance with the planned provisions of the contract constituting Annex 4 to this SWZ.
