Description:
4.1. The subject of the contract is the implementation of the task entitled: "Delivery of one -off specialist equipment for the hemodynamics and angiocardiography laboratory for the needs of SP ZOZ CSK UM in Łodź" - II - 6 packages, in accordance with the assortment and quantities specified in the price form constituting Annex 2to SWZ. The subject of the contract was presented in detail in Annex 3 to SWZ - technical parameters.
Package no: 1 electronic for examination of vascular echocardiography IVUS - 40 items (to the "bank" - 5 pcs)
Package no: 2mikrocewnik for CTO two -chains in the RX/OTW system - 10 pcs.
Package No. 3inflator (syringe) with pressure reading up to 30 ATM. - 100 pcs.25 pieces (5 pcs to the "bank")
The order was divided into 49 parts. The contractor may submit an offer for any number of parts.
4.2. For packages No. 1, 6, as part of the delivery, the Contractor undertakes to create a "Bank of equipment for angiography and hemodynamics", hereinafter referred to as the "bank" inthe headquarters of the Ordering Party, i.e. in the hospital complex at ul. Pomorska 251 (CKD) in Łodź.
4.3. In the "bank", the Contractor within 2 weeks from the date of the contract will deliver and entrust the ordering goods free of charge in the quantities specified in Annex 4 to the contract template.
