- 1. The subject of this order is the delivery of an operational microscope for the National Oncology Institute of Maria Skłodowska - Curie -State Research Institute of the Branch in Gliwice
2. Description of the subject of the contract and the terms of the contract are included in the attachments:
1) parametersTechnical, assortment and price specification-Annex 2 to SWZ,
2) of the planned provisions of the contract, which will be entered into the content of the contract-Annex 4 to SWZ.
3. The contracting authority does not allow the possibility of submitting partial offers
4. Offers not containing the full range of the subjectOrders will be rejected.
5. The Employer allows the possibility of submitting equivalent offers - the equivalence description was included in Chapter IV SWZ.
6. The contracting authority requires the contractor to submit a bid bond and secure the proper performance of the contract
7. The contracting authority does not provide for an electronic auctionand supplementary orders.
8. Each Contractor must attach the means of evidence to the offer, which are to confirm that the offered subject of the contract corresponds to the requirements specified by the Ordering Party:
1) declaration of compliance and/ or CE certificate (depending on the class of the medical device)and relevant statements (if required) in accordance with the 2017/745 Regulation on medical devices (MDR) or with the Council Directive 93/42/EEC (MDD) together with later transitional provisions.
