Description:
1. The subject of the order is: Supplies of orthopedic implants for the Central Clinical Hospital of the Medical University of Lodz at ul. Pomorska 251
2. The order was divided into 2 parts.
3. A detailed description of the subject of the order is attached as Appendix No. 1 to the SWZ - Assortment form - pricing All references in the content of the document to standards, the ordering party allows solutions equivalent to those described, and such a reference is accompanied by the words "or equivalent", in accordance with Art. 101 section 4 of the Public Procurement Law.
4. Any use of terms and proper names in the documentation is for example purposes only and serves to determine the class and quality of the goods. The Ordering Party allows the submission of equivalent offers.
By equivalent, the Ordering Party understands the subject of the order with the same parameters and functionality as those indicated in the description of the subject of the order. If other equivalent products are proposed please add the proposed material (as item A) in the assortment and price form. Failure to meet even one of the technical requirements or minimum parameters of the subject of the order will result in rejection of the offer.
5. The contractor is obliged to complete the order on terms and conditions described in the contract template constituting Annex No. 2 to the SWZ, depending on which part it submits an offer for.
