Description:
The subject of the order are: Delivery of orthopedic medical devices for SPZOZ WSS No. 3 in Rybnik.
The supply specification is included in Annex 1.1, ie "price forms".
Other terms of the contract are specified in Annex 2 to the specification of the terms of the contract, i.e. the designed provisions contracts that will be entered into the content of the contract /contract provisions. This attachment includes 2 different contract templates (constituting Annexes 2.1 and 2.2) - depending on the assortment covered by a given package. The rules set out in the contracts also indicate for individual packages, among others duties in the scope of method and dates of delivery, including: in terms of deposit and lending.
Agreements in the scope of resolved packages will be concluded after completing these proceedings for a period of 24 months.
The contracting authority informs that in accordance with Art. 139 of the Public Procurement Law will first examine and evaluate the offers, and then the contractor's subjective qualification, whose offer was the highest assessed in a given package, in the scope of the lack of grounds for exclusion and meeting the conditions for participation in the proceedings.
