Description:
The subject of the order is the delivery of orthopedic products to SPZOZ in Łukow.
A detailed description of the subject of the contract is included in Annex 2 to SWZ - the assortment and price form, which, after completing the contractor in the offered, selected scope will constitute an inseparable part of the offer and contract, If you choose a given offer as the most advantageous.
Order scope: The order was divided into 17 parts:
Package No. 1, No. 2, No. 3, No. 4, No. 5 - Implants,
Package No. 6 - orthopedic materials,
Package No. 7 - bone substitute,
Package No. 8 - bone cement + rinsing system,
Package No. 9, No. 10 - semi -material materials,
Package No. 11 - endoscopic materials,
Package No. 12 - one -time staplery and staples,
Package No. 13 - SEVERY AND HEOSTATIC MATERIALS;
Package No. 14 - hernial nets and vascular prostheses,
Package No. 15 - special semi -material,
Package No. 16 - SEVERS MATERIALS,
Package No. 17 - orthopedic materials.
The contracting authority does not specify the number of part of the order to which the Contractor may submit an offer or maximum number of packages (parts) to which the order may be awarded to the same contractor.
The estimated quantities of the order specified in the attachment may be shifted quantitative and intention, including between packages, reduction or suspension depending on the demand of the contracting authority, due to the variability of the quantity qualified for the treatment of patients and the size of contracts signed with the National Health Fund.
