• Description:
• 1. The subject of the order is the supply of PCR tests and kits for the Veterinary Inspection of the Provincial Veterinary Inspectorate in Szczecin, to the extent specified in Annex No. 1 to the SWZ, divided into 26 parts. 2. Detailed scope and requirements for each part of the procedure is specified in the SWZ and Annex No. 1 to the SWZ, which is the Product and Price Form. 3. The detailed method of implementing the subject of the order is specified in Annex No. 2 to the SWZ constituting the Designed Provisions contracts. 4.SUBJECT MEASURES OF EVIDENCE: 1) The Ordering Party informs that in each part of the procedure, in order to confirm compliance of the offered supplies with the requirements, features or criteria specified in the description of the subject of the order, the Ordering Party requires that If the contractor offers equivalent products, he/she submitted with the offer: (a)samples of equivalent products offered relating to lots 1, 3, 4, 5, 7 and 25 of the procedure; b) primary validation reports of equivalent products offered, in the language Polish or translated into Polish with the term, among others: manufacturer's name, catalog number, product parameters, indicating which part of the procedure and which item within this part the documents concern; c) quality specification, inspection certificate quality or other equivalent document which clearly shows that the offered equivalent product meets the requirements of the description of the subject of the contract, in Polish or translated into Polish with the specification...
