Description:
1. The subject of the order is the supply of personal insulin pumps with the ability to administer a bolus remotely and with an application controlling a closed loop for patients of the Hospital Diabetes Clinic of the University Clinical Hospital. Fryderyk Chopin in Rzeszow.2. Detailed description of the items of the order, quantity supplies and requirements were included in the Price and Offer Form (Appendix No. 1 to the SWZ) and in the Description of the subject of the order, a list of required parameters (Appendix No. 2 to the SWZ), which are an integral part of the SWZ.3. The scope of the order includes the delivery of the items of the order, in the quantities specified in the Form. price and offer - Appendix No. 1 to SWZ.4. The Ordering Party allows solutions equivalent to those described in the description of the subject of the order as described in SWZ.5. ORDER COMPLETION DEADLINE: 1). Successive deliveries according to the current needs of the Ordering Party for a period of 12 months from the date of conclusion of the contract.2). The ordering party allows the possibility of changing the validity period of the contract in the event of failure to complete the entire subject of the contract (basic order) during the contract period due to the implementation of lower orders.
