1. The subject of the procedure is the award of the contract for successive delivery of pharmaceutical products and medical oxygen, in the range of thirty packages for the Clinical Hospital dr. J. Babiński SPZOZ in Krakow, according to the assortment listed in Annexes No. 1.1 - 1.30 to SWZ, constitutingAt the same time, price forms: Packages I XXVIII Drugs Annexes 1.1 - 1.28 to SWZ Package XXIX - Food of special medical purpose - Annex 1.29 to SWZ, XXX Medical Oxygen Package Annex 1.30 to SWZ 2.- where part of the order is a given package according to division above. 3. The Contractor in each position of price forms - in which the Employer requires, provides: a responsible entity or manufacturer/manufacturer or manufacturer or responsible entity/manufacturer or manufacturer/manufacturer; GTIN code; trade nameProduct in accordance with the requirements of individual price forms. 4. The subject of the contract will be delivered to the Employer in accordance with the conditions set out in the Regulation of the Minister of Health of 13.03.2015. on the requirements of good distribution practice. 5. The contractor is obliged to deliverthe subject of the contract together with information containing the manufacturer's indication, a description containing the manufacturer's indications as to the properties of the subject of the contract, the safety rules and expiration dates.
