Description:
The subject of the order is the supply of preparations for enteral and parenteral nutrition along with accessories and formula milk in accordance with the division into 3 tasks.
The detailed description of the subject of the order is:
Appendix No. 1 to SWZ - "Assortment and price form"
Detailed terms and conditions the implementation of the subject of the order is additionally specified in the "Contractual Conditions", which are Annex No. 5 to the SWZ.
The requirements that the pumps must meet are included in Annex No. 1 to the SWZ.
Detailed conditions and rules for the implementation of the subject of the order are additionally specified
Contractual Terms and Conditions constituting Annex No 5 to SWZ "Contractual Terms".
Delivery date:
1) according to current needs, each time within a maximum of 3 business days from the date
placing an order.
Series and expiration date on the unit packaging and on the invoice.
Expiry date:
1) for the supply of enteral nutrition products with accessories - half term
validity for a given product;
2) for the supply of parenteral nutrition preparations with accessories 12 months.
The contractor will deliver the subject of the order to the HOSPITAL PHARMACY Warehouse.
7. The offered preparations must be approved for marketing and use in the Republic of Poland
and possess all legally required registration documents confirming this fact in accordance with the Act of September 6, 2001. Pharmaceutical Law (TJ Journal of Laws
from 2021 pos. 1977, as amended) if applicable.
