Description:
The subject of the contract is the supply of preparations for enteral nutritional
and parenteral with accessories and modified milk as divided into
64 tasks.
A detailed description of the subject of the contract is:
Annex 1 to SWZ-"Assortment and Centive Form"
Detailed conditions and rules The implementation of the subject of the contract is additionally determined by "contractual terms", which is annex 5 to SWZ.
2. The applicant also requires, for the period of the contract, the lease for:
1) tasks No. 1 - 28 pumps, for enteral use both by the patient's bed as
and portable.
2) Task No. 4 - 60 pumps intended for enteral nutrition in hospital or home conditions.
3) Task No. 6 - 25 pumps intended for parenteral nutrition in hospital or home conditions.
4) The requirements that the pumps must meet are included in Annex 1 to SWZ.
3. Detailed conditions and rules The implementation of the subject of the contract is additionally determined by
Contractual conditions being Annex 5 to SWZ "Contractual conditions".
ATTENTION !!!!!!
Additional requirements for tasks 22-23:
1) medication not recorded in Poland is allowed
2) drugs that do not have leaflet in Polish, translated leaflet are allowed The buyer should be made available to Polish.
3) Delivery condition - a target import application accepted by the Minister of Health.
4. Delivery date:
1) according to the current needs, each time within a maximum of 3 business days from the date
placing an order.
