Description:
1. The subject of the order is the supply of program medicines and medicines various for the Provincial Hospital John Paul II in Bełchatow.
2. CODE AND NAME according to a common order dictionary (CPV):
33.60.00-6 Pharmaceutical products
3. Description, size and scope of the subject of the contract:
1) Ordering Partydivided the subject of the order into 33 parts (packages No. 1 - 33), as described in the price form according to Annex 2 to SWZ.
2) Other requirements regarding the performance of the contract and the principles of cooperation between the contracting authority and the contractor specify the planned provisions of the contracton a public contract, constituting Annex 3 to SWZ.
