Description:
1. The subject of the contract is the supply of reagents and disposable articles for the needs of the hospital laboratory for a period of 24 months.
2. The order was divided into 19 packages
1. To confirm the compliance of the services offered, it requires the submission of the following together with the offer listed belowin question:
1) a statement that the offered assortment was admitted and placed on the market in accordance with the provisions of the Act of 7 April 2022 on medical devices and has current declarations of compliance and certificates of the notified unit and will be made availableAt each request of the Ordering Party after signing the contract - the content of the statement entered in the offer form constituting Annex 1 to SWZ
2) a document confirming that the antibiotic concentration in the disc is in the range of 90-125% of the determined concentration, as determined by the DIN- or equivalent standard - applies toPackage No. 16 item 1-5
3) Document of the manufacturer of antibiotic discs regarding the criteria for approval of the antibiotic concentration ranges contained on the discs according to DIN standard or equivalent - applies to package No. 16 item 1-5
4) positive opinion of Korld or an equivalent center for antibiotic discs (at leastfrom 2020), which is a confirmation of the quality of the discs obtained and their safety in the diagnostic process- applies to package No. 16 item 1
5) positive opinion of Korld or equivalent center for package No. 4
