- 1. The subject of the order is: SUPPLY OF REAGENTS, CONTROL MATERIALS AND CONSUMABLES IN THE FIELD OF CLINICAL BIOCHEMISTRY AND HEMOSTASIS, TOGETHER WITH THE LEASE OF ANALYZERS for the Provincial Specialist Hospital. Blessed Virgin Mary in CzΔstochowa.
2. The description of the subject of the order was included in the forms assortment and prices, constituting Appendix No. 2 to the SWZ. Other conditions regarding the execution of the order are specified in Annex No. 6 to the SWZ, constituting the draft provisions of the contract (PPU).

Part No. 2 - Reagents for hemostasis tests with the lease of 3 automatic analyzers: 2 analyzers (basic and reserve) for the Laboratory Diagnostics Department at ul. Bialska 104/118 and 1 analyzer for the Medical Diagnostic Laboratory at ul. PCK 7 for a period of 24 months.
