1. The subject of the contract is the supply of reagents, control, useful materials along with the lease of analyzers, diagnostic tests, substrates, genetic tests for UCK WUM for a period of 12, 24 and 36 months divided into 13 parts
2 to SWZ- assortment and price form, Annex 1-5 (allergens) to Annex 2.1 to SWZ- a form of required technical conditions (applies to Part 1), Annex 2.2 to SWZ- Form of technical and assessed parameters (applies to Part 2), Annex No. 2.3 to SWZ-SWZ-form required technical conditions (applies to Part 4), Annex 2.4 to SWZ - form of the required technical and assessed conditions (applies to Part 5), Annex 2.5 to SWZ - form of the technical parameters of the analyzer (applies to Part 8), Annex 2.6 to SWZ - technical parameter form computer (applies to part 8);
