Description:
1. The subject of the tender is the selection of a contractor who will provide the ordering party with supplies consisting of successive deliveries by the contractor to the District Hospital. Dr. Tytus Chałubiński in Zakopane, ul. Kamieniec 10 reagents for testing critical parameters and necessary accessoriesand consumables for analyzers along with the simultaneous lease of two analyzers referred to in point 4.9 of the SWZ.
2. The Ordering Party stipulates that the delivered subject matter of the order referred to above must have a valid marketing authorization required by law and one hundreduse in Poland, allowing for medical use in accordance with the intended purpose, including in particular:
1) CE certificate with a declaration of conformity,
2) ISO 13485 certificate (or equivalent).
The Ordering Party informs that whenever the SWZ refers to the ISO certificate referred tomentioned e.g. above in point 2, the contractor may have equivalent documents issued by entities based in another Member State of the European Economic Area. Moreover, a contractor who, for reasons beyond his control, is unable to obtain the certificate referred to abovesee point 2, may have another document regarding quality assurance or environmental management measures, respectively, confirming the contractor's use of quality assurance measures consistent with the required quality assurance standards or environmental management measures equivalent to those...
