3Clinical Hospital Fryderyk Chopin in Rzeszow.
2. A detailed description of the subjects of the contract, the number of deliveries and the requirements are included in:
a) price and offer form - reagents, reference blood cells, useful and control material (Annex 1 to SWZ);
b) price and offer form- lease of analyzers and a manual set (Annex 2 to SWZ);
c) list of parameters required (Annex 4 to SWZ);
d) a list of evaluated parameters (Annex 5 to SWZ),
which is an integral part of SWZ.
3. The scope of the order includes the delivery of subjects in quantitieslisted in price and offer forms for a period of 36 months.
4. The contracting authority does not allow balance offers. The subject of the order is the supply of reagents, blood cellsexemplary, useful and control materials for performing markings in the field of transfusion immunology as well as lease of automatic analyzers and a manual set for the clinical Department of Laboratory Diagnostics of the Laboratory Diagnosis of the University Clinical Hospital Fryderyk Chopin in...
